What Are wRVUs?
Work RVUs are physician work relative value units used in the Medicare Physician Fee Schedule. They describe physician work intensity and time, not guaranteed reimbursement or compensation.
How APP Productivity Differs
APP productivity attribution varies by payer, setting, supervision model, documentation, split/shared rules, and employer policy. APP Mode keeps physician wRVUs visible and adds an educational APP estimate rather than replacing the physician value.
Assistant-at-Surgery Estimates
For surgical CPT codes, APP Mode uses a 13.6% assistant-at-surgery equivalent. That reflects the Medicare concept that PA, NP, and CNS assistant-at-surgery services are calculated as 85% of the 16% assistant-at-surgery amount when the service is allowed and medically necessary.
E/M Productivity Estimates
For E/M codes such as 99202-99205, 99211-99215, 99221-99223, 99231-99233, 99242-99245, and 99252-99255, APP Mode uses an 85% Medicare NPP equivalent. This is not an employer productivity rule.
